when I go to my date and time properties, the options to automatically synch
time are disabled. I am running ISA server sbs2003. Is it being blocked. Can
I enable it, is it safe to enable?
Or are there other/better options for keeping the server clock up to date.

Use: Net Time
From a command Prompt on the server
net time /?
net time /setsntp: < Public time server such as time.windows.com >

Ensure UDP port 123 is open on all firewalls. Note: UDP -not- TCP.
