You can create additional fields, choose the "add to default view" option
and they will show up on the upload page. Unfortunately, there isn't an easy
way to do this with the default Date, Keyword, Description field. I have no
idea why you don't get this same option for the fields that are already
there. I haven't tried it myself, but you could probably edit the
upload.aspx page for this library using FrontPage 2003 to add these fields.
