I have one user account that i've created...it was fine at one point but now
it appears to lockout everytime the user logs on. If I check the security log
it says sucessful logon but the user has no creditenials to visit the web
and/or access exchange server and if I log off/log on the user account is
locked out again.
This is a continous process, could someone please help and explain why this
is happening?

Hi April. If the machine is XP Pro I'd suspect the user has somehow save the
incorrect credentials against their account. Go into Control Panel, User
Accounts, Advanced, Manage passwords. Remove anything related to your
domain. Other than passports, you should be safe to have nothing at all in
there. I've had this a couple of times....

Has the user recently changed their password? Are they mapping network
drives when they log in that require a username and password? Most likely
somewhere they have cached an incorrect password. Do what Jeff suggests and
also check for what I suggested.

Yes the user did change their password...my problem is resolved but i dont
understand why this happened?

A lot of times users will map a network drive and select "Reconnect at
logon". It is required they supply a username and password. So everytime
they login in order to connect that network drive it needs to be
authenticated. If a wrong password is supplied it will not connect and it is
the same as trying to login with a incorrect password. Thus after a few
times Windows will clasify it as a Intrusion Detection and lock the account
after so many failed attempts.
