Hi.

I have Remote Desktop exposed to the internet on a 2003 standard server and
wonder how "secure" this really is? Is the connection encrypted? ...

Yes, RDP connection is encrypted.

As Mike said the RDP uses encryption and I have used it quite a bit over the
internet as is. You can increase security if you can go through a VPN
connection - particularly L2TP which will only allow computers with a
trusted certificate to access the VPN server. Also configuring your firewall
to accept connections from only authorized IP addresses will also increase
security, though that may not be an option if the client is roaming or has
dynamic public IP address. --- Steve

In addition, you can adjust the encryption strength requirements
of RDP itself, along with tweaking other settings.
These are found in the properties of the RDP-tcp connectoid
in the right pane when running the TS Configuration admin tool.
