I've seen several SSL questions in the forum regarding items on a page
throwing the good ole "this page contains both secure and unsecure items" -
but no real resolution.

I am getting the message described on the Shared Documents page of an
external SharePoint site with SSL enabled. Regardless of whether I pick Yes
or No to display the unsecure items, the page looks the same. When I look at
the source of the page, I don't see any items that are using just http or
pulling from a different address.

There is only on item on the list -- a pdf file, which strangely, SharePoint
does not have an icon for. I would suspect that the issue is related to this
item - primarily because there is another document library on the site that
does not throw the message when opened. That document library doesn't have
any items currently. (However, if I click Explorer View, the message does
appear...)

Any ideas?

ps. It definitely is related to the pdf document -- I removed it, and the
page loaded cleanly. I put it back in, and the message reappears.
SharePoint doesn't pull these icons from the client machine, does it?

It shouldn't. Can you right click on the image and look at the properties of
it and see where the image is located?

And to double check you have configured your alternate portal url?

Also, have you tried assigning the icon for pdf document?
