I am installing ultrasound on my win2003 server which is also a DC. I have
successful installed it and can connect to the Database but the provider is
not getting installed. When i try to install it it go from Install(pending)
to Install(queued). I have tried to manually run the USProvider.msi and it
starts to run then rollsback. Can anyone give me some insight or something
to try.

Could you grab %windir%\USProvider.log (log file is written out by the
windows installer) and %windir%\USSetAcl.log (log file written while
modifying the WMI namespace) from that machine ? They may contain useful
data.

The USProvider.log file shows a successful deploy on the original install
date... there is nothing there on the attempts to deploy it manually after
that. In the USSetAcl.log file this is all that was in there
Usage:
USSetACL <namespace> <machine account>
for example:
USSetACL MicrosoftUltrasoundV1 NetBIOSName$@Domain
